Re-imagining Planned Care together

This year we are looking to change and improve our planned care services throughout Northamptonshire through the Re-imagining Planned Care Programme.
 
Planned care, also known as elective care, is the provision of services with planned appointments, such as an outpatient appointment or surgery, in a place like a GP surgery or hospital.
  
Working together with our system partners, patients, carers and our local communities, and learning from experience and best practice, we believe we can address current challenges and significantly improve outcomes and care for people in Northamptonshire.​

Why are we doing this?

There have already been innovations and improvements made within specific hospital services, however we know there continues to be challenges including:
  • Long waits with patient’s conditions deteriorating during delays
  • Care not always delivered in a local or convenient location for patients
  • Limited coordination of care, meaning multiple appointments that patients may struggle to make, often with more than one service involved
  • Patients not feeling like they understand or getting what they need from some of the appointments they attend
  • Inequalities in access, experience and outcomes.

How can you be involved?

We want to hear from the many different perspectives in our local community. Your views will help to inform and share the design of future planned care services so they will work for you.
