Dementia Care

Our dedicated dementia specialist nurse helps to support patients living with dementia and their loved ones with a range of different services. 

The service operates on a referral system, therefore if specialist support is required, a member of staff can make a referral to the dementia nurse.

Services includes:

  • Behavioural and Psychological symptoms support in dementia care
  • Person-Centred care planning support
  • Family/carer support and intervention (education, complex situations, new baseline and managing expectations)
  • Support with complexity involving safeguarding principles
  • End of Life dementia care support
  • Complex discharge support
  • Any patient with Younger Onset Dementia

Support within the Hospital

  • Carers have the right to continue to provide support to the patient with dementia while they are in hospital see John's Campaign for more information.
  • Concessionary parking - Speak to the nurse in charge if your loved one is likely to be in hospital for a prolonged stay.
  • All about me patient profile (Word document) - This can be printed and completed and brought into hospital. It will support staff to provide person centred care for the person living with dementia. Otherwise please ask a member of staff for a booklet to complete.
