How many 'drug errors' have been made by staff in the past 12 months (April 2010 - April 2011) and how many were made in the previous 12months (April 2009 - April 2010?

I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do not hold the information that you have requested centrally, in the format that you have required.

 

We have estimated that to locate and collate all the information that you have requested would exceed the appropriate limit as set out in the Freedom of Information and Data Protection (Appropriate Limit and Fees) Regulations 2004 as it would require us to manually extract the information manually from records.

 

Therefore, to locate the record and search for the information requestedwould take an average of 1.5 minutes for each of 1057 records, in excess of 26 hours at cost of £660. The limit set down by regulations for public authorities is £450, which amounts to a maximum of 18 hours of search and retrieval time charged at £25 per hour, the hourly rate also specified in the regulations.  Under Section 12 of the Freedom of Information Act 2000 the Trust is therefore not obliged to provide this information to you.  Please be aware that this is not an exemption which requires us to consider the application of the public interest test

 

In the event that you decide to refine the terms of your request I would advise you that the Trust might also rely on the engagement of other exemptions under the Freedom of Information Act.
