The Trust does not hold this information centrally. We have established that it would take more than 18 hours to locate all the information you haveasked for, as it would require us to locate and manually extract the information from each Department within the Trust as each Department have their own forms for tasks specific to the service being provided. Therefore weconsider that the exemption offered to us in S.12 of the Freedom of Information Act 2000 relatingto the cost of compliance is applied, since we are of the opinion that the costs which we areallowed under the Act exceeds £450. As you will be aware this is not an exemption which requiresus to consider the application of the public interest test.

We are currently exploring the possibility of creating electronic forms utilising Microsoft Infopath on the Trust’s Sharepoint system. We expect some forms to be available electronically during the next 12 months.
